New Patient: Registration

Please arrive 15 minutes prior to your scheduled appointment time to complete medical and dental information forms. These forms can also be downloaded in pdf format by clicking on "Registration Forms." If you have dental insurance, remember to bring your insurance card with you.

New Patient: First Visit

Step 1: Our friendly administrative personnel will greet you when you walk in the door. Please bring a copy of your dental insurance card. You may be asked for additional information at that time so it is always a good idea to arrive early.

Step 2: The hygienist will make a complete series of radiographs and provide a dental cleaning.

Step 3: The doctor will then see you for a thorough examination. He will review the radiographs made by the hygienist. The doctor will share with you his findings and discuss any areas that need attention.

Step 4: A treatment plan will be developed for you to follow. In some instances, a second visit to discuss your treatment is required.
